Waiver of Premium
| If an employee becomes disabled prior to reaching age 60, coverage continues while that
person is disabled and ceases when the disability ends or when the insured reaches normal
Social Security retirement age. |

Conversion
| If an individual should have part or all of his/her coverage terminated, the individual may
apply to convert his/her insurance to an individual policy, subject to the terms of the group
policy. If applied for within 31 days of termination, no evidence of insurability is required. |
|
|
|
Portability Provision
| If employees leave their jobs before normal retirement age, they can continue their
life coverage. Portability is standard for Voluntary Plans and optional for Basic
Plans. It is available for both employees and their dependents. Benefit levels may
vary and are based on qualifying events. |

|
Age Reduction Formula
| Our standard reduction schedule for Life and Accidental Death & Dismemberment coverage is
35% of the original amount at age 65, 35% additional at ages 70 and 75, and 25% of the previously
adjusted amounts at ages 80, 85, 90, and 95. Benefits normally terminate at retirement. Other
schedules are available upon request. |

OPTIONAL BENEFITS

Accidental Death and Dismemberment
An amount equal to the basic life benefit, unless requested differently, will be paid should death or
certain dismemberment occur within 365 days following accidental bodily injury. The actual amount payable
is based on the severity of the loss. The only exclusions are for death or dismemberment due to war or acts
of war, suicide or intentionally self-inflicted injuries, committing a felony, bodily or mental infirmities
or disease, disabilities due to intoxication or drug use (unless prescribed or by a physician), or injury
sustained while on full-time active duty as a member of the armed forces of any country or international
authority.

Spouse & Dependent Life Insurance
| Available in a variety of
schedules and benefit amounts. Employees have the option of providing coverage on their spouse and
any unmarried children under the age of 19 (under age 25 if full-time student). Eligible dependents
must be performing normal daily activities and must not be confined in a hospital on effective date
of coverage. |

Accelerated Death Benefit
| This product feature is standard in all
plans and provides a benefit of up to 80% of the employee’s life insurance specified amount to a maximum
benefit of $100,000. Higher maximums are available upon request. Although some states may mandate a
lower minimum amount, an individual must be covered for at least $20,000 to be eligible for this
